1.0 Preliminaries
1.1. Dale Nadeau opened the meeting at 7:30 PM.
1.2. Roll Call: Dale Nadeau, Jim Saart and Health Agent Sam Wong. Dan Ehntholt was absent.

2.0 Hearings and Appointments
2.1 7:30 PM - Septic System Discussion 21 Worcester Ave., Nicole Benander, Al Sousa & Steve Poole
Sam Wong explained that the homeowner Nicole Benander called for this meeting. Wong provided the Board with an update. In 2003, he investigated a septic system failure and sited the homeowner to have the system repaired. One year later Ms. Benander hired engineer Steve Poole to design the system. Sam noted that there are 7 houses on this one property. Poole designed a system for 4 houses and noted that the 3 other houses/cottages have their own system.
In June 2004, the original septic design plan was submitted to the Board of Health. The Board approved the plan October 2005. One month later Poole asked for revisions. Sam explained that the first installer left the job. Al Sousa explained that when he got involved there was an exposed gas line. Wong stated that in September 2006 he issued a probation order Sousa Services because system was never completed and the Board of Health was never called to do any inspections.
Sam stated that the system includes the singular system. The items remaining to complete the system are as follows:
1) A report from the manufacturer of the Singular system stating that the unit has been installed correctly
2) A service contract with the homeowner
3) An as-built from the engineer.
Sam noted that the deadline for the original Order was May 2005 and the case is still open.
Steve Poole submitted the inspection form from singular stating the system was installed properly.
Steve Poole also presented an as-built plan however; he said it was not complete.
Steve Poole also had a draft of the service agreement/contract.
Nicole Benander said that her property is a mess and the tenants do not have a place to park. She continued that the installer did not finish the system and refuses to finish the job. Sousa said he is owed money on this job.
Poole said that the system is functioning and they need to submit an as-built plan. He noted that the manufacturer of Singular does offer a service contract.
Benander asked the Board to revoke Sousa’s license.
Nadeau said that it is determined that the system is functioning and there is no health issue. He said that the three documents needed for closure do not have anything to do with Sousa.
Jim Saart stated that the issues between Sousa and Benander are not issues for the Board he believes it is non-productive for this meeting. The Board said all of the three items must be submitted to the board for closure.
2.2 Massage Establishment License – The Hudson Natural Health Center
Proctor explained that Hands for Healing has changed ownership. Dale Nadeau made a motion to issue the Massage Establishment License to the Hudson Natural Health Center.
